Information that we will collect includes the following, Name, E-mail Address, Contact Lens Information, Prescription Information, Date of Birth, Phone Number, Address, Appointment Time Requests, Doctor Preference, and any other information required for appointment scheduling. Credit Card information will be required when ordering contact lenses. All information collected is done so by you, the visitor filling out the forms described above. No personal information listed above will be gathered in any other way.
